Health Leads ensures that, in addition to primary care needs, people have access to housing, good nutrition, safety, and public programs.
At Heartland Health Outreach, our care philosophy focuses on those same needs: serving the whole person through nutrition counseling, case management, safe housing, and social service access in addition to primary, dental, and mental health care. No one can be healthy without a roof over their heads and good food on their plates.
